Completed contracts must be received at least 1 business day prior to your service start date. Service start date must be a Monday - Friday, excluding holidays.
Required Documents
- Legible copy of Government Issued photo ID and be 18 years of age or older.
- Lease agreement for all individuals who rent (or rent to own) at the residence.
- Contact information that applicant can be reached at during our office hours.
- Any required deposit must be paid before service will be turned on.

- Lifenet: Emergency ambulance service. The program cost is $6 per month and billed on your utility statement.
