Application for Residential Utility Services

Completed contracts must be received at least 1 business day prior to your service start date. Service start date must be a Monday - Friday, excluding holidays.

Required Documents

  1. Legible copy of Government Issued photo ID and be 18 years of age or older.
  2. Lease agreement for all individuals who rent (or rent to own) at the residence.
  3. Contact information that applicant can be reached at during our office hours.
  4. Any required deposit must be paid before service will be turned on.

  6. Lifenet: Emergency ambulance service. The program cost is $6 per month and billed on your utility statement. If you do not want this service you must complete an opt out form and send it to the address on the top of the form. Western Payne County Ambulance Trust Authority Benefit Changes Form (PDF)
